Your heading should include your full name and degree on the first line, your current title and the department you belong to on the second line, and the organization you are connected to on the third. Keep everything centered.
Include the full date when this version of your CV was made. As much as possible, the date should be within three months of your submission.
Include your complete and accurate contact information, but avoid putting in personal information such as marital status or date of birth. Stick to your address, phone number, fax number, email address and languages you are proficient in.
